Cramming patients into an already busy schedule while working though lunch and after office to write notes and document in an electronic medical record (EMR) is an all too common story. Can you do this every day for the rest of your life?

Remember medical school, when you felt that your career would land you on top of the world, make you rich, and let you take exotic Instagram-worthy vacations?

The harsh reality is that this dream is becoming harder to achieve. Reimbursement for medical services continues to plummet. For example, the Medicare Physician Fee Schedule conversion factor is lower than it was in 2002! Physician burnout is rising. And to make things worse, most clinicians are leaving money on the table for work they are doing because they don't understand the complex documentation and coding guidelines.

our story

After residency, I felt unstoppable. However, I quickly found myself forced into solo practice. I was renting out office space in the cheapest strip mall I could find. My wife would work the front desk and we'd both team up to clean the office on the weekend, just to keep overhead low. Being an excellent clinician just wasn't enough to truly succeed in medicine. There was more to the hustle than just taking care of patients. I had to become a businessman to run a lean medical office.

Then it hit me. To leverage medical billing to maximize reimbursement, I first had to learn the rules of medical coding. It started with scribbling ICD and CPT codes on the back of notecards. This quickly evolved into self publishing my first book on General Surgery Coding in 2015.

When I started my medical practice, I wanted to sleep well at night knowing that I would get paid appropriately for every patient I saw and procedure I performed. Twenty years later, I go to work because I enjoy it, not because I feel like a slave to productivity metrics.

why isn't everyone a pro at medical coding?

You'd think that after going through 10+ years of medical training, we would emerge with an iron clad understanding of medical coding. But the majority of clinicians are never taught appropriate medical coding and billing. What's left are clinicians who are slogging through the day, getting complex spreadsheets every month from administration about how they need to cram more patients into the day while maintaining "value metrics."

The truth is that everyone wants to be good at medical coding, but resources are lacking. Outdated blogs aren't comprehensive or specialty specific. On the other hand, specialty specific programs are bulky and expensive. No one has the time to sift through resources and streamline the process. It's impractical to dive into lengthy websites after patient encounters to figure out what the appropriate code for the encounter is.

And that's how we end up in our current situation. A workforce of clinicians who are undercoding, out of ignorance.

THe math

Let's say that you run a busy office clinic and see 20 patients on average per day. You base your coding on "gut feeling" and instinct. You pick the code that is a mid-level code just so that you don't get audited for undercoding or overcoding. You spend a ton of time on documentation, even at the end of the day. Your coding bell curve peaks on a level of 3.2. Now, let's say you learn how to document efficiently and code accurately. You cut down your documentation time by 20% and shift your coding bell curve peak on a level of 3.6. You're documenting less but getting paid more...BY FOLLOWING THE RULES!

Over the course of 1 year, if you took 6 weeks of vacation, saw only Medicare patients (RVU conversion factor of 32), and you have a 70%/30% mix between established/new patients, you would earn an extra $42,000 per year with a coding curve peak shift from just 3.2 to 3.6.
